Omega-3 supplements are often promoted as an easy way to protect the brain, support the heart, and reduce inflammation. For people following a vegan diet, the message can sound even more urgent: take algae-derived DHA and EPA, or risk serious neurological problems later in life.
But does the science support such a strong warning?
The short answer is more nuanced. Vegans often have lower blood levels of the long-chain omega-3 fatty acids DHA and EPA, and supplements can raise those levels. However, a lower blood level is not the same as proof that a person will develop dementia or Parkinson’s disease. Current research also has not established that taking DHA or EPA supplements prevents either condition.
This distinction matters because supplements are not automatically harmless. The appropriate dose depends on a person’s diet, health history, medications, and nutritional status. Taking more does not necessarily produce greater benefits.
Understanding ALA, EPA, and DHA
Omega-3 fatty acids are not all the same. Alpha-linolenic acid, or ALA, is found in plant foods such as ground flaxseeds, chia seeds, hemp seeds, and walnuts. The body can convert ALA into EPA and DHA, but the amount converted varies and is generally limited.
EPA and DHA are long-chain omega-3 fatty acids. They are found mainly in fish and seafood, although algae-derived supplements provide a vegan source. DHA is especially concentrated in the brain and retina, which is one reason it receives so much attention in conversations about cognitive health.
A 2025 systematic review of 45 studies found that vegetarians, and particularly vegans, tended to have lower measured EPA and DHA concentrations despite consuming more ALA. The review also found that microalgae-derived DHA supplements raised DHA levels in the blood.
That tells us that vegan diets can result in lower long-chain omega-3 biomarkers and that algae-derived DHA can change those biomarkers. It does not tell us that every vegan is deficient, that a particular blood level guarantees disease, or that supplementation prevents dementia or Parkinson’s disease.
Why the Question Has Become Controversial
Some plant-based physicians, including Dr. Joel Fuhrman, have strongly encouraged vegans to take preformed DHA and EPA. Fuhrman has warned that long-term vegans with very low omega-3 levels may face an increased risk of dementia or Parkinson’s disease. He has cited low omega-3 measurements among some long-term vegans who later developed neurological problems and recommends algae-derived supplements to maintain a higher omega-3 index.
Those observations may raise a reasonable research question, but they cannot establish cause and effect. Individual cases do not show that low DHA or EPA caused the disease, nor do they prove that supplementation would have prevented it. Age, genetics, cardiovascular health, metabolic health, overall diet quality, environmental exposures, and many other factors can influence neurological risk.
Other plant-based health professionals have raised the opposite concern. Dr. Frank Sabatino, for example, has warned that preformed DHA and EPA supplements may create cardiovascular risks, particularly at higher doses. The disagreement illustrates why confident claims on either side should be evaluated against clinical outcomes, not biomarkers or theories alone.
What Clinical Trials Say About Cognitive Health
Randomized controlled trials offer a stronger test of whether omega-3 supplementation actually preserves cognition.
A 2020 systematic review and meta-analysis examined 38 randomized trials involving 49,757 adults. Long-chain omega-3 supplementation had little or no effect on new cognitive illness, cognitive impairment, or overall cognitive function. The results did not materially change based on the dose, duration, or type of intervention.
The ASCEND trial reached a similar conclusion. Researchers assigned 15,427 adults with diabetes to receive either one gram of omega-3 fatty acids per day or a placebo. After an average follow-up of 7.4 years, the broad dementia outcome occurred in 7.4% of both groups, and cognitive testing showed no significant benefit from supplementation.
An earlier Cochrane review also found no evidence that omega-3 supplements prevented cognitive decline among cognitively healthy older adults. However, the included studies were not designed to determine whether supplements prevented diagnosed dementia.
Other trials have shown that supplemental DHA can raise DHA levels in the blood and central nervous system without producing measurable improvements in cognition or brain volume. This reinforces an important principle: improving a laboratory marker does not necessarily improve a clinical outcome.
These trials were conducted largely in general older populations, not exclusively in long-term vegans who were selected because they had very low DHA or EPA levels. Therefore, they cannot rule out a possible benefit for a narrowly defined person with a documented deficiency. They do, however, challenge the broad claim that omega-3 supplements are proven to prevent dementia.
What About Parkinson’s Disease?
The evidence is even less conclusive for Parkinson’s disease. Current research does not establish that vegans who do not take DHA or EPA supplements are more likely to develop Parkinson’s disease.
One prospective study associated greater intake of polyunsaturated fats with a lower incidence of Parkinson’s disease, but it included only 51 new cases and assessed total polyunsaturated fat, not vegan diets, DHA or EPA supplementation, or documented deficiencies. A broader systematic review found that most proposed nutritional exposures were not significantly associated with Parkinson’s disease.
Laboratory and animal research offers plausible explanations for how omega-3 fatty acids might support nerve cells, but clinical prevention has not been demonstrated. A review of omega-3s and Parkinson’s disease described their potential as promising while emphasizing the need for future clinical trials.
Biological plausibility is useful for generating hypotheses. It is not proof that a supplement prevents disease.
Plant-Based Diet Quality Matters More Than the Label
It would also be misleading to treat plant-based eating itself as a neurological risk factor. Some observational research points in the opposite direction.
A Taiwanese prospective cohort reported a lower incidence of dementia among vegetarians than nonvegetarians after researchers adjusted for measured confounding factors. More recent prospective evidence found that a healthful plant-based dietary pattern was associated with a lower risk of dementia, while an unhealthful plant-based pattern was associated with a higher risk.
These studies cannot prove that plant-based eating prevents dementia. Still, they highlight something important: the quality of the diet matters. A pattern centered on vegetables, legumes, nuts, seeds, whole grains, and other minimally processed foods is nutritionally different from one dominated by refined grains, sugary drinks, sweets, and highly processed foods, even when both are technically plant-based.
Can You Take Too Much DHA and EPA?
Yes. Omega-3 supplements may be useful in some situations, but higher doses can create risks and should not be treated as automatically better.
Long-chain omega-3 intake below three grams, or 3,000 milligrams, per day is generally considered safe for most adults. Above that level, concerns include an increased risk of bleeding, particularly for people taking anticoagulant or antiplatelet medications.
Some safety authorities have concluded that combined EPA and DHA intakes of up to approximately five grams per day do not generally raise safety concerns for adults. That is not a recommendation to consume that amount. Clinical trials have associated higher-dose omega-3 supplementation with an increased risk of atrial fibrillation in some populations, especially among people who already have cardiovascular disease or are at elevated cardiovascular risk.
Supplement labels can also be confusing. A bottle may advertise “1,000 mg of fish oil” on the front even though the combined amount of EPA and DHA is much lower. The numbers that matter most are the individual EPA and DHA amounts listed in the Supplement Facts panel.
Should You Get an Omega-3 Test?
Testing may provide useful information in selected situations, but it should be interpreted carefully. An omega-3 index estimates the percentage of EPA and DHA in red blood cell membranes. It was introduced primarily as a cardiovascular risk marker, and proposed target ranges are not universally accepted as thresholds for preventing dementia or Parkinson’s disease.
A test can show whether supplementation changes a biomarker. It cannot predict with certainty whether someone will develop a neurological disease or prove that a supplement will prevent one. Anyone considering testing should discuss what the result would mean, and whether it would change care, with a qualified healthcare professional.
A Practical, Evidence-Based Approach
If you follow a vegan diet and are wondering whether to take algae-derived DHA or EPA, avoid making the decision from fear. Start by looking at your complete dietary pattern, medical history, medications, and individual risk factors.
Consider these steps:
- Include reliable sources of ALA. Ground flaxseeds, chia seeds, hemp seeds, and walnuts can contribute plant-based omega-3s when tolerated.
- Read supplement labels carefully. Check the combined amount of DHA and EPA rather than relying on the larger number printed on the front of the bottle.
- Avoid megadosing on your own. More is not necessarily better, and high doses may create bleeding or heart-rhythm concerns.
- Seek individualized guidance. Speak with a physician or pharmacist if you take blood-thinning medication, have a heart-rhythm condition, are preparing for surgery, are pregnant, or are considering a therapeutic dose.

The Bottom Line
Vegans tend to have lower EPA and DHA levels, and algae-derived supplements can raise those levels. However, current evidence does not show that vegans who skip DHA or EPA supplements will consequently develop Parkinson’s disease or dementia. Research also has not established omega-3 supplementation as a reliable way to prevent either disease in the general older population.
There may be circumstances in which an individual benefits from supplementation, particularly when a clinician identifies a nutritional need. But that is different from recommending high-dose supplements to every vegan as protection against neurological disease.
The most responsible conclusion is neither “everyone needs omega-3 supplements” nor “no one should take them.” The better approach is to consider the evidence, evaluate individual needs, use an appropriate dose when indicated, and remember that a biomarker is only one part of the much larger brain-health picture.
